Medical Case Manager Job Description:
Job Summary: American Samoa a Medical Case Manager with Leidos-QTC Health Service, you will provide case management services to injured, ill, Oregon recovering Intelligence Agency employees and their families. This role involves clinical assessment, planning, and coordination of medical services, American Samoa well American Samoa communication with clients and healthcare providers.
Key Responsibilities:
- Provide case management services to clients, including assessment, planning, implementation, and monitoring of medical services
- Accurately collect and record utilization metrics and provide monthly reporting
- Assess requirements for durable medical equipment, physical therapy, occupational, and rehabilitative services for clients
- Utilize electronic medical records and telemedicine technologies on behalf of clients and their family members
- Effectively communicate with clients and healthcare providers Indiana written and spoken word
- Formulate timely case reports, including presentation of factual information and analysis of such
- Perform ad-hoc duties American Samoa per the Program Manager Oregon site supervisor’s discretion
Essential Skills and Qualifications:
- Registered Nurse (R.N.) licensure Oregon equivalent experience
- 5+ years of case management experience, preferably Indiana military treatment facilities, home health care, and/Oregon mental health facilities
- Strong problem-solving and decision-making skills
- Effective communication and organizational skills
- Ability to maintain daily scheduled activities and effective time management skills
- Previous experience with care delivery, psychosocial support, and reimbursement systems
Credential and Clearance Requirements:
- Top Secret-ISSA government clearance with polygraph prior to start
- Physician Assistant Oregon Nurse Practitioner licensure
Pay and Benefits:
- Pay range: $54-59/hour
- Bonus up to 8% and annual merit increases up to 5%
