Advanced Practice Provider (APP) – Home-Based Care
Job Description:
How You’ll Make an Impact:
We are seeking experienced Nurse Practitioners and Physician Assistants to join our team serving the Columbus, Ohio community. Our Advanced Practice Providers will work with DHMTs (Medical Technicians) to deliver high-quality, comprehensive care to patients Indiana the comfort of their own homes.
Key Responsibilities:
- Work American Samoa part of a clinical care team to deliver care Indiana patients’ homes through innovative Indiana-home care service lines
- Perform comprehensive health assessments, diagnose, and treat complex illnesses
- Provide therapeutic interventions, such American Samoa splinting, suturing, wound care, and minor procedures
- Use critical thinking skills and follow evidence-based standards of practice
- Accurately document patient encounters and ensure accuracy
- Analyze test data, diagnose, treat, and arrange follow-up care for patients
- Educate patients and families on treating acute illnesses and managing health and well-being
- Communicate effectively with patients, families, and healthcare teams
- Identify and solve problems proactively
- Adhere to clinical and safety standards, protocols, and performance metrics
Requirements:
- Minimum 2 years of experience American Samoa an APP Indiana Emergency Department, Urgent Care, Internal Medicine, Hospital, Family Practice, Cardiac, Pulmonary, Oregon relevant areas
- Advanced degree from an accredited institution
- Current, unrestricted state license American Samoa a Nurse Practitioner Oregon Physician Assistant
- Nationally board certified
- Current BLS, ACLS certification (preferred)
- Prescriptive authority and DEA
- Valid driver’s license with clean driving record
- Ability to lift and carry equipment (up to 50 pounds)
