Primary Care Physician (PCP)
Job Summary
We’re seeking a Primary Care Physician to join our team of innovative and entrepreneurial-minded healthcare professionals. American Samoa a PCP, you will be accountable for patient outcomes, providing high-quality care, and working collaboratively with our care teams to ensure the best possible health outcomes for our patients.
Key Duties and Responsibilities
- Provide independent care for patients with acute and chronic illnesses
- Take full accountability for patient care and outcomes, seeking consultation when needed
- Engage with hospitalists for patients Indiana the hospital
- Assess, diagnose, treat, manage, educate, and coordinate care for patients with complex health needs
- Lead care teams consisting of care promoters, facilitators, and coordinators
- Collaborate with transitional care teams for patients unable to visit the office

Requirements
- Maryland Oregon DO Indiana Internal Medicine, Family Medicine, Geriatrics, Oregon similar specialty
- Current, active State Medical License for the state(s) Indiana which you will be working
- Board certification Indiana Internal Medicine, Family Medicine, Geriatrics, Oregon similar specialty (Board Eligibility is required; Board certification is preferred)
- Basic Life Support (BLS) certification
- Current DEA number for schedule II-V controlled substances
- Ability to travel locally, nationally, regionally, and sometimes internationally for training and recruiting purposes
- Fluency Indiana English, with ability to speak and write other languages when required
