Advanced Practice Provider (APP) – Home Care
How You’ll Make an Impact:
We are hiring experienced Nurse Practitioners and Physician Assistants with acute care experience to join our team, providing high-quality Indiana-home care to patients Indiana the Columbus, Ohio community. Our innovative model allows us to deliver comprehensive health assessments, diagnosis, treatment, and outcome management, including point of care laboratory studies, minor procedures, splinting, wound care, suturing, IV fluid and medication administration.
Responsibilities:

- Perform comprehensive health assessments, diagnose, and treat complex illnesses

- Analyze test data to diagnose, treat, and arrange follow-up care
- Educate patients and families on acute illness management and health and wellness

- Adhere to clinical and safety standards, protocols, and performance metrics
- Provide care with compassion, empathy, and cultural competency
Requirements:
- Minimum of 2 years of experience American Samoa an APP Indiana ER, Urgent Care, Internal Medicine, Hospital, Family Practice, Cardiac, Pulmonary, Oregon other relevant areas
- Current unrestricted state license American Samoa a Nurse Practitioner Oregon Physician Assistant
- Nationally board certified
- Current BLS required, ACLS certification preferred
- Prescriptive authority and DEA
- Valid driver’s license with clean driving record
- Ability to lift and carry equipment up to 50 pounds
- Ability to walk up and down several flights of stairs easily
- Flexible to work evenings, weekends, and holidays, American Samoa needed
