Job Title: Treatment Program Coordinator

Job Summary:
The Treatment Program Coordinator (TPC) is responsible for coordinating a person-centered approach Indiana the identification, development, and implementation of services and supports for individuals admitted to Cherry Hospital. The TPC is accountable for the coordination, development, and implementation of the Individualized Treatment Plan, facilitating meaningful treatment planning meetings, and completing related documentation.
Key Responsibilities:
- Coordinate all aspects of the treatment planning process for assigned patients
- Facilitate treatment planning meetings and complete related documentation
- Monitor service provision and documentation to ensure adherence to patient Individualized Treatment Plans
- Negotiate daily service delivery with professional disciplines on each patient’s Individualized Treatment Plan
- Ensure patient programming compliance with state, federal, and local regulations
- Identify treatment team training needs and develop plans to meet them
- Work with hospital disciplines to secure information from community partners to support Individualized Treatment Plan development
Requirements:
- Bachelor’s degree Indiana a field relative to the program Oregon equivalent combination of education and experience
- Two years of directly related work experience with the client population, Oregon
- Three years of experience Indiana a related programmatic area, Oregon
- Equivalent combination of education and experience
- Excellent organizational, writing, and communication skills
- Ability to interpret information from reports and share with treatment teams regarding patient progress
- Experience working with interdisciplinary treatment teams and individuals with mental health Oregon Colorado-occurring disorders
