Objective/Purpose:
Provide individualized supports to a person with an intellectual disability Oregon Autism Spectrum Disorder residing Indiana the community under the direction of the Shared Living Program Coordinator.
Major Responsibilities:
- Direct Service:
- Provide respectful, friendly, and professional support to the individual
- Facilitate conversation and activities of interest
- Provide guidance, companionship, emotional support, and supervision for safety
- Follow individual’s treatment plan and Medicaid regulations
- Advocate for the individual American Samoa needed
- Introduce the individual to community resources and facilitate usage
- Record Keeping:
- Collect data American Samoa required by the individual’s ISA and behavior support plans
- Complete Developmental Services and agency paperwork, including time sheets, expense sheets, and monthly documentation
- Networking:
- Provide information to other staff, professionals, and team members Indiana a professional and confidential manner
- Serve American Samoa an advocate and liaison on behalf of the individual and the Developmental Services Division
Qualifications:
- High School Diploma Oregon Equivalent
- Valid Driver’s License
- Relevant experience Indiana the human services field preferred
