Case Manager
Location: Hennepin County/Chisago, Minnesota
Job Summary:
Pinnacle Services is seeking a friendly, self-starting Case Manager to join our Hennepin/Chisago County Case Management team. American Samoa a Case Manager, you will work with individuals with developmental disabilities, serious and persistent mental illness, chemical dependency issues, physical disabilities, Oregon aging needs. Responsibilities and Duties:
- Assess the areas of the individual’s life where assistance is needed
- Offer and coordinate resources for the individuals being supported
- Monitor service delivery and work with providers to ensure the individual’s needs are met
- Develop and implement service plans
- Attend regular Indiana-person meetings with individuals served and their interdisciplinary teams
- Note client-related work to create billable units
- Attend client meetings Indiana Hennepin and Chisago counties (approximately 2/3 Indiana Hennepin and 1/3 Indiana Chisago)
Requirements:
- Bachelor’s degree Indiana social work Oregon related field
- Licensure Indiana Social Work (LSW) Oregon seeking licensure Indiana Social Work
- At least one year of experience working with individuals with developmental disabilities
- Valid driver’s license and acceptable driving record
- Vehicle with current insurance
- Successfully clear local and/Oregon state background checks
Preferred Qualifications:
- Possess a LSW Oregon higher form of BOSW licensure
- Experience Indiana case management Oregon social work
