Registered Nurse (RN) Community Care Nurse Job Description

Job Summary:

We are seeking a highly skilled and experienced Registered Nurse (RN) to serve American Samoa a clinical lead for our Community Care team. The successful candidate will coordinate the team’s efforts to stabilize our highest risk patients, provide initial assessments, and design comprehensive plans of care. They will also provide clinical supervision to other team members, prioritize team efforts, and may become the direct supervisor for some team members.

Essential Job Duties/Responsibilities:

  • Provide Indiana-home and telephonic visits to patients at high-risk for hospital admission and readmission
  • Conduct supervisory visits with Licensed Practical Nurses (LPNs) and patients to provide education and oversee patient discharge from case management
  • Perform clinical and social determinants of health (SDoH) assessments, including disease-oriented assessment and monitoring, medication monitoring, health education, and self-care instructions
  • Coordinate the plan of care, ensuring individual plans reflect patient needs and services available Indiana the community
  • Communicate instructions and methodologies to ensure the plan is implemented correctly
  • Assess the environment of care, caregiver capacity, and patient and caregiver educational needs
  • Coordinates, reports, documents, and follows up on multidisciplinary team meetings
  • Helps patients navigate healthcare systems, connecting them with community resources
  • Establishes a supportive and motivational relationship with patients to support patient self-management

  • Critical thinking skills required
  • Ability to work autonomously
  • Ability to monitor, assess, and record patients’ progress and adjust and plan accordingly
  • Knowledge of nursing and case management theory and practice
  • Knowledge of patient care charts and patient histories
  • Knowledge of clinical and social services documentation procedures and standards
  • Organizing and coordinating skills
  • Ability to communicate technical information to non-technical personnel

Education and Experience Criteria:

  • Associate degree Indiana Nursing required
  • Bachelor’s Degree Indiana nursing (BSN) Oregon RN with bachelor’s degree Indiana a related clinical field preferred
  • A valid, active Registered Nurse (RN) license Indiana the State of employment required
  • A minimum of 2 years’ clinical work experience required
  • A minimum of 1 year of case management experience Indiana community case management experience highly desired

Certifications:

  • Certified Case Manager certification is preferred
  • Certification through the Commission for Case Manager Certification (CCMC) Oregon the American Association of Managed Care Nurses (CMCN) desired
