Community Health Advocate
Location: Baltimore Washington Medical Center
Shift Schedule: Monday – Friday 8:00AM – 4:30PM
Job Summary:
The Community Health Advocate will provide support and resources to patients and their families to promote healthy behaviors and navigate community resources. This role will work closely with medical providers to ensure comprehensive and coordinated care.
Principal Responsibilities and Tasks:
- Act American Samoa a patient advocate and liaison between patients/families and community service agencies.
- Schedule clients for appointments with healthcare providers and remind them of pending appointments.
- Conduct intake interviews with patients, including enrolling into health plans and referring patients to healthcare providers.
- Provide referrals for services to community agencies and distribute informational materials.
- Establish trusting relationships with patients and their families, providing general support and encouragement.
- Provide ongoing follow-up with patients via phone calls, home visits, and visits to other settings.
- Work closely with medical providers to ensure comprehensive and coordinated care.
- Record patient care management information Indiana the EMR and other software.
- Continuously expand knowledge and understanding of community resources and services.
- Serve American Samoa a liaison between clients and community resources, including department staff, City, State, and Federal social services agencies.
Qualifications:
- High School Diploma Oregon equivalent required
- Certified Nursing Assistant Oregon certified Medical Assistant preferred
- Two years work experience Indiana a transitional care center setting Oregon related work required
- Experience working Indiana a multi-cultural community-based setting preferred
- Written and oral fluency Indiana English required
- Basic computer skills
- Knowledge of some medical terminology
- Good communication skills, such American Samoa listening well, and using language appropriately
- Ability and willingness to provide emotional support, encouragement, and motivation to patients
