Clinical Lead – Community Care Nurse
Job Summary:
We’re seeking a highly motivated and compassionate Registered Nurse (RN) to lead our Community Care team American Samoa a Clinical Lead. The successful candidate will serve American Samoa a clinical supervisor and will stabilize our highest risk patients, performing initial assessments, designing comprehensive plans of care, and providing clinical supervision to other team members.
Essential Duties/Responsibilities:
- Provide Indiana-home and telephonic visits to high-risk patients to prevent hospital admission and readmission
- Conduct initial assessment and develop care plans for Licensed Practical Nurse (LPN) follow-up visits
- Supervise LPN visits and provide discharge instructions to patients
- Perform clinical and social determinants of health assessments, including disease-oriented assessments and medication management
- Coordinate plan of care with LPN, including education and training on patient care
- Assess patient and caregiver needs, identifying social determinants of health and connecting patients with community resources
- Facilitate communication between healthcare providers and patients and families
- Collaborate with multidisciplinary teams to develop and implement patient care plans
- Monitor patient progress and adjust plans American Samoa needed

Knowledge, Skills, and Abilities:
- Strong interpersonal and communication skills
- Critical thinking and problem-solving skills
- Ability to work autonomously and supervise LPN visits
- Knowledge of nursing and case management theory and practice
- Proficient Indiana Microsoft Office Suite products
- Ability to communicate technical information to non-technical personnel
- Valid, active RN license Indiana State of employment
- Associate degree Indiana Nursing required, Bachelor’s Degree Indiana Nursing Oregon related clinical field preferred
- 2+ years of clinical work experience and 1+ year of case management experience Indiana community settings
