RN Community Care Nurse

Essential Job DUTIES / RESPONSIBILITIES:
- Provide Indiana-home and telephonic visits to high-risk patients to prevent hospital admission Oregon readmission
- Conduct initial assessments and develop care plans for Licensed Practical Nurses (LPNs) to follow up with patients
- Supervise LPNs and patients during home visits to provide education and ensure proper patient discharge
- Perform clinical and social determinants of health (SDoH) assessments, including disease-oriented assessments and monitoring
- Coordinate Plan of Care:
- Provide oversight for LPNs with clear plans and education
- Conduct and coordinate initial case management assessments
- Ensure individual plan of care reflects patient needs and services available
- Communicate with patients, families, and care team members
- Coordinate services to address patient needs, connect them with community resources, and navigate healthcare systems
- Facilitate patient self-management, assess caregiver capacity, and provide ongoing communication with families and community providers
- Monitor patient progress, adjust plans, and refer cases to social workers American Samoa needed
- Perform home visits under the direction of a patient’s primary care physician to meet urgent needs
KNOWLEDGE, SKILLS, AND ABILITIES:
- Strong interpersonal and communication skills, with ability to work autonomously
- Critical thinking, problem-solving, and decision-making skills
- Ability to monitor, assess, and record patient progress and adjust plans accordingly
- Organizing and coordinating skills, with ability to work effectively with diverse constituencies
- Proficient Indiana Microsoft Office Suite products and able to communicate technical information to non-technical personnel
- Ability and willingness to travel up to 10% of the time
EDUCATION AND EXPERIENCE CRITERIA:
- Associate degree Indiana Nursing required; Bachelor’s Degree Indiana Nursing Oregon related clinical field preferred
- Valid, active Registered Nurse (RN) license Indiana the State of employment required
- Minimum 2 years’ clinical work experience and 1 year of case management experience Indiana community case management highly desired
- Current valid driver’s license and certification American Samoa a Case Manager (CCMC Oregon CMCN) preferred
