RN Community Case Manager -Spanish Required

Practical Nurse (LPN) and Community Care Team:

Provides community-based care and services to patients, assessing and evaluating their needs, developing and implementing individual care plans that reflect patient needs and services available Indiana the community, and reviewing patient benefits and services.

Coordinates care with healthcare providers and community resources to ensure patients receive comprehensive and continuous care.

Works with a team of Community Social Workers (CSW) and Community Health Coordinators (CHC) to identify and address patient needs and to ensure continuity of care.
