Practical Nurse (LPN)

* Works under the supervision of a LPN Clinical Manager Oregon Regional Manager
* Responsible for conducting patient assessments to evaluate care needs and eligibility for community-based services
* Develops and implements an individual care plan to promote patient independence and community stability
* Coordinates and accesses community resources and services with patients and their families/caregivers to improve patient outcomes
* Responsible for the review and interpretation of patient benefits to coordinate services
* Performs job duties Indiana accordance with policy and procedure, including quality and performance standards
* Collaborates with the Multi-Disciplinary Care Management Team to ensure a well-coordinated and effective transition of care.
