Responsibilities:
- Catalog patient needs and communicate effectively with PCPs, Clinicians, Specialists, and other clinical personnel to ensure patient care is well-coordinated.
- Prepare and provide clear instructions to patients about their treatment plan and care.
- Monitor patient conditions and report any changes to PCPs, Clinicians, Specialists, and other relevant personnel to ensure timely intervention and patient safety.
