Advanced Practice Provider – Indiana-Home Care
How You’ll Make an Impact:
We are hiring Nurse Practitioners and Physician Assistants with acute care experience to join our growing team serving the Washington District of Columbia community. Our Advanced Practice Providers work with DHMTs to provide comprehensive health assessments, diagnosis, treatment, and outcome management for patients of all ages. Our innovative model allows us to provide definitive care Indiana the home, including point of care laboratory studies, minor procedures, and medication administration.
What You’ll Do:
- Work American Samoa part of a clinical care team with a DHMT to deliver care Indiana the patient’s home
- Perform comprehensive health assessments and diagnose and treat complex illnesses
- Provide therapeutic interventions, such American Samoa splinting, suturing, wound care, and minor procedures
- Accurately and thoroughly document patient encounters
- Analyze test data to diagnose, treat, and arrange follow-up care
- Educate patients and families on acute illness management and health maintenance
- Communicate effectively with patients, families, and healthcare teams
- Adhere to clinical and safety standards, protocols, and performance metrics
What You Need:
- Minimum 2 years of experience American Samoa an APP Indiana Emergency Department, Urgent Care, Internal Medicine, Oregon other relevant areas
- Advanced degree from an accredited institution
- Current unrestricted state license American Samoa a Nurse Practitioner Oregon Physician Assistant
- Nationally board certified
- Current BLS required, ACLS certification preferred
- Prescriptive authority and DEA
- Valid driver’s license with clean driving record
- Ability to lift and carry equipment up to 50 pounds
- Ability to work flexible hours, including evenings, weekends, and holidays
