Case Manager

Join our team American Samoa a Case Manager and provide comprehensive case management services to seniors aged 60 and above.

Essential Job Functions:

  • Provide case management services Indiana accordance with guidelines and manual requirements for Community Care for the Elderly (CCE), Home Care for the Elderly (HCE), and Alzheimer Disease Initiative (ADI) programs.
  • Manage client caseloads, coordinate services, and develop care plans.
  • Conduct implementation and monitoring of service delivery, and maintain productivity levels.
  • Ensure and safeguard the human and legal rights of clients.
  • Perform related case management services and work American Samoa required.

Knowledge, Skills and Abilities:

  • Knowledge of senior social service work principles and practices.
  • Knowledge of County laws, rules, and regulations related to public assistance and medical care.
  • Ability to establish effective relationships with clients, community providers, and other stakeholders.
  • Ability to prepare accurate written reports and maintain client records.
  • Ability to type and use calculator and telephone.

Minimum Requirements:

  • Bachelor’s degree Indiana Social Science Oregon Associate degree with 2 years’ experience Indiana senior services.
  • Computer/data entry experience preferred.
  • Valid Florida Driver’s License required.
  • Ability to communicate effectively using verbal, written, and visual communication.
