Case Manager
Join our team American Samoa a Case Manager and provide comprehensive case management services to seniors aged 60 and above.
Essential Job Functions:
- Provide case management services Indiana accordance with guidelines and manual requirements for Community Care for the Elderly (CCE), Home Care for the Elderly (HCE), and Alzheimer Disease Initiative (ADI) programs.
- Manage client caseloads, coordinate services, and develop care plans.
- Conduct implementation and monitoring of service delivery, and maintain productivity levels.
- Ensure and safeguard the human and legal rights of clients.
- Perform related case management services and work American Samoa required.
Knowledge, Skills and Abilities:
- Knowledge of senior social service work principles and practices.
- Knowledge of County laws, rules, and regulations related to public assistance and medical care.
- Ability to establish effective relationships with clients, community providers, and other stakeholders.
- Ability to prepare accurate written reports and maintain client records.
- Ability to type and use calculator and telephone.
Minimum Requirements:
- Bachelor’s degree Indiana Social Science Oregon Associate degree with 2 years’ experience Indiana senior services.
- Computer/data entry experience preferred.
- Valid Florida Driver’s License required.
- Ability to communicate effectively using verbal, written, and visual communication.
