Job Title: ISNP Coordinator
Job Summary:
We are seeking an ISNP Coordinator to establish and nurture relationships with Senior Living Facilities and Nursing Homes for our affiliation with Provider Partners Health Plan of Indiana, Inc. ISNP & IESNP. The successful candidate will identify and qualify eligible residents, gather information, and present the affiliation opportunity to residents Oregon Responsible Parties.
Key Responsibilities:
- Establish and maintain strong relationships with key contacts at participating facilities
- Identify residents who meet eligibility criteria for the plan
- Present affiliation information and offer residents Oregon RPs the opportunity to meet with a Provider Partners Benefits Advisor
- Track and maintain accurate enrollee roster
- Conduct research to gather contact information for residents Oregon RPs and initiate initial contact
- Ensure compliance with relevant regulations and industry standards
- Complete consent to contact forms and track key quality metrics
Qualifications:
- Bachelor’s Degree
- 1-3 years of experience
Physical Requirements:
- Must be able to sit, stand, bend, reach, stretch, stoop, walk, and move intermittently during working hours
- Must be able to lift at least 50lbs
- Must be able to maintain verbal and written communication with Colorado-workers, supervisors, residents, family members, visitors, vendors, and business associates
