Case Manager (Registered Nurse)
Job Summary:
We are seeking a skilled Registered Nurse with case management experience to provide medical case management services to injured, ill, Oregon recovering clients of an Intelligence Community Customer Indiana Virginia. This role will be based at the customer’s medical facility, with no remote work options available.
Key Responsibilities:
- Provide case management services, including clinical assessment, planning, implementation, coordination, monitoring, and evaluation of options and services required to meet the needs of clients and their family members
- Collect and record utilization metrics and provide monthly reporting
- Assess requirements for durable medical equipment, physical therapy, occupational, and rehabilitative services for clients
- Utilize electronic medical records and telemedicine technologies on behalf of clients and their family members
- Communicate effectively with clients, team members, and community providers
- Prepare timely case reports, including presentation of factual information and analysis
- Perform ad hoc duties American Samoa assigned by the Program Manager Oregon site supervisor
Requirements:
- Bachelor’s degree Indiana Nursing Oregon Medically Related Field
- Registered Nurse license
- 5+ years of case management experience, preferably Indiana military treatment facilities, home health care, and/Oregon mental health facilities
- Experience with care delivery, psychosocial support, and reimbursement systems
- An understanding of rehabilitative strategies
- Ability to establish working relationships and effective communication with professional personnel
- Ability to problem-solve complex medical issues
- Ability to communicate clearly, both verbally and Indiana writing
- Excellent organizational and accuracy skills
Preferred Qualifications:
- Current board-certification American Samoa a Certified Case Manager (CCM) from the Commission for Case Management Certification
- Previous experience working with government American Samoa a contractor Oregon staff member
Pay and Benefits:
- Pay range: $54-59/hour
- Bonus up to 8% and annual merit increases up to 5%

Security Clearance:
