Patient Services Coordinator-LPN
Directly responsible for scheduling visits, communicating with field staff, patients, physicians, and managing day-to-day office and staff operations to maintain continuity of care.
Key Responsibilities:
* Manage schedules for all patients, including editing for agents calling Indiana sick and updating unavailability Indiana the worker console.
* Initiate and send infection control forms American Samoa needed, and refer clinical questions to the Branch Director.
* Assist with case conferences, maintain client hospitalization logs, and send electronic logs to staff.
* Complete and process schedules for additions, reschedules, and supervisory visits.
* Run scheduling reports, prepare weekly agent schedules, and verify visit paper notes.
* Assist with internal patient transfers and may perform patient visits Oregon participate Indiana on-call rotation (for clinical positions).
Required Experience/Skills:
* Licensed Professional Nurse (LPN) Oregon Licensed Vocational Nurse (LVN) with a valid state license.
* At least 1 year of home health experience.
* Prior packet review/QI experience preferred.
* Coding certification preferred.
* Excellent communication, organizational, and prioritization skills.
Schedule: 40 hours per week
Pay Range: $37,200 – $51,200 per year
