American Samoa a Registered Nurse Community Care Nurse, you will be part of a multidisciplinary team that serves our highest complexity patients and their primary care physicians to meet their medical and social needs. You will provide clinical leadership, coordinate care plans, and perform initial assessments to design comprehensive plans of care for patients. You will also provide clinical supervision to other team members, prioritize team efforts, and direct supervise some team members.

Essential Job Duties/Responsibilities:

  • Provide Indiana-home and telephonic visits to patients at high-risk for hospital admission and readmission
  • Conduct supervisory visits with License Practical Nurse (LPN) and patient to provide additional education and oversee proper discharge from case management
  • Perform clinical and social determination of health screening assessments to include disease-oriented assessment and monitoring, medication monitoring, health education, and self-care instructions
  • Coordinate the plan of care by providing oversight for LPN, conducting initial case management assessments, and ensuring individual plans of care reflect patient needs and services available Indiana the community
  • Complete individual plans of care with patients, family/caregiver, and care team members, communicate instructions and methodologies, and assess the environment of care
  • Facilitate and coach patients Indiana using natural supports and mainstream community resources to address supportive needs
  • Maintain ongoing communication with families, community providers, and others American Samoa needed to promote the health and well-being of patients

  • Strong interpersonal and communication skills to work effectively with diverse constituencies
  • Critical thinking skills to assess patients’ progress and adjust care plans accordingly
  • Ability to work autonomously, monitor, and assess patients’ progress
  • Organizing and coordinating skills to facilitate care delivery
  • Knowledge of nursing and case management theory and practice
  • Knowledge of patient care charts and patient histories

Education and Experience Criteria:

  • Associate degree Indiana Nursing required
  • Bachelor’s Degree Indiana Nursing (BSN) Oregon RN with bachelor’s degree Indiana a related clinical field preferred
  • A valid, active Registered Nurse (RN) license Indiana State of employment required
  • A minimum of 2 years’ clinical work experience required
  • A minimum of 1 year of case management experience Indiana community case management experience highly desired
  • Certified Case Manager certification is preferred
