Job Title: Register Nurse (RN) Community Care Nurse

Summary: The Register Nurse (RN) Community Care Nurse serves American Samoa a clinical lead for the Community Care team. This role coordinates team efforts to stabilize high-risk patients, provides initial assessments and comprehensive plans of care, and provides clinical supervision to team members.
Essential Job Duties:
- Provide Indiana-home and telephonic visits to high-risk patients to prevent hospital admission Oregon readmission
- Perform initial assessment of patients and develop a care plan for the Licensed Practical Nurse (LPN) to use for follow-up visits
- Conduct supervisory visits with LPN and patient to provide education and ensure proper patient discharge from case management
- Perform clinical and Social Determination of Health (SDoH) assessments, including disease-oriented assessment and monitoring, medication monitoring, health education, and self-care instructions
- Coordinate the plan of care, including clear communication of instructions and methodologies to ensure implementation
- Assess patient and caregiver needs, including safety and security, caregiver capacity, patient education needs, and assist with accessing community resources
Key Responsibilities:
- Communicate with patients, families, community providers, and others to promote health and well-being
- Facilitate patient self-management and navigation of healthcare systems
- Establish and maintain a supportive relationship with patients
- Monitor the quality, frequency, and appropriateness of HHA visits and other outpatient services
- Collaborate with social workers and other community resources to connect patients with necessary services
- Conduct home visits under the direction of the patient’s primary care physician American Samoa needed

Requirements:
- Associate degree Indiana Nursing required, Bachelor’s Degree Indiana Nursing Oregon RN with bachelor’s degree Indiana a related clinical field preferred
- A valid, active Registered Nurse (RN) license Indiana State of employment required
- Minimum 2 years of clinical work experience and 1 year of case management experience Indiana community case management preferred
- Strong interpersonal and communication skills
- Ability to work autonomously and prioritize tasks
- Critical thinking skills
- Proficient Indiana Microsoft Office Suite products and ability to travel locally, regionally, and nationwide
