Clinical Leader – Community Care Team
We’re a dynamic team committed to improving the health and well-being of our patients and ourselves. American Samoa a Clinical Leader on our Community Care team, you will be part of a multidisciplinary service that includes Registered Nurses, Licensed Practical Nurses, Community Social Workers, and Community Health Coordinators working together to provide comprehensive care to our patients.
Primary Responsibilities:
- Provide Indiana-home and telephonic visits to patients at high-risk for hospital admission Oregon readmission, conducting initial assessments and developing care plans.
- Conduct supervisory visits with Licensed Practical Nurses (LPNs) and patients, providing guidance on patient education and discharge planning.
- Perform clinical and social determinants of Health (SoH) assessments, including disease-oriented assessments, health education, and social services.
- Coordinate the Plan of Care, providing oversight for LPNs and ensuring clear and effective communication with patients and families.
- Develop and implement individual care plans that reflect patient needs and services available Indiana the community.
Requirements:
- Associate degree Indiana Nursing, with a valid and active Registered Nurse (RN) license.
- Strong communication and interpersonal skills.
- Ability to work autonomously and make critical thinking decisions.
- Basic computer skills, including Microsoft Office Suite and other standard software.
- Valid drivers license, with the ability to travel locally, regionally, and nationwide (up to 10% of time).
- Ability to speak and write fluently Indiana English, with bilingualism a plus.
Preferred Qualifications:
- Bachelor’s Degree Indiana Nursing Oregon a related clinical field.
- Valid, active Certified Case Manager (CCM) certification.
