Requirements:
- Associate degree Indiana Nursing (ASN) Oregon Bachelor’s Degree Indiana Nursing (BSN), preferred
- Valid, active Registered Nurse (RN) license Indiana the state of employment

- Certified Case Manager certification preferred
- Strong interpersonal and communication skills, with the ability to work autonomously
- Ability to monitor, assess, and evaluate patient progress and adjust care plans
