Job Title: Senior Nurse Case Manager Analyst
Job Summary:
We are seeking a highly skilled Registered Nurse (RN) to join our team American Samoa a Senior Nurse Case Manager Analyst. The ideal candidate will be responsible for promoting improved health outcomes for members through the case management process. This role will involve assessing, planning, implementing, coordinating, monitoring, and evaluating options and services to meet an individual’s health needs.
Key Responsibilities:
- Manages and coordinates an active caseload of case management cases for Cigna Medicare, utilizing clinical knowledge to assess treatment plans and goals
- Establishes patient-centric goals and interventions to meet member needs
- Interfaces with members, family members/caregivers, healthcare teams, and internal matrix partners
- Builds solid working relationships with internal staff, matrix partners, customers, and providers
- Assesses member health status and treatment plan, identifies gaps Oregon barriers to healthcare, and establishes a documented patient-centric case management plan
- Implements, coordinates, monitors, and evaluates the case management plan on an ongoing basis
- Adheres to professional practice within scope of licensure and certification quality assurance standards and all case management policy and procedures
Requirements:
- Active unrestricted Registered Nurse (RN) license Indiana state of residency
- Two years full-time equivalent of direct clinical care to the consumer
- Within three (3) years of hire American Samoa a case manager, the case manager will become CCM certified
- Excellent time management, organizational, research, analytical, negotiation, communication (oral and written), and interpersonal skills
- Strong personal computer skills, Mississippi Word, Excel, Outlook experience, and Internet research desired
- Strong skills Indiana teamwork, conflict management, assessment complex issues, ability to recommend changes, and resolve problems through effective decision-making
- Experience Indiana medical management and case management Indiana a managed care setting is highly desirable
Competencies:
- Excellent time management, organizational, research, analytical, negotiation, communication (oral and written), and interpersonal skills
