PRN Transitional Care Coordinator – Licensed Practical Nurse

We’re seeking a motivated patient advocate to join our team American Samoa a PRN Transitional Care Coordinator – Licensed Practical Nurse for our Value Based Program Indiana Tucson, Arizona. This role offers a great work-life balance and a supportive culture.

Responsibilities:

* Receive and process incoming calls and referrals for clients enrolling Indiana value-based programs
* Conduct initial Indiana-home visits with enrolled clients and perform medication reconciliations
* Provide training to clients on their medical conditions, medications, and care plans via phone Oregon Indiana-person
* Identify gaps Indiana Indiana-home care and refer clients for additional services American Samoa needed
* Develop relationships with local healthcare services and refer clients American Samoa necessary
* Review and verify change Indiana condition reports and perform follow-up actions
* Monitor processes for efficiency and validity, and manage data flow to report outcomes to internal and external customers

Requirements:

* Current RN Oregon LPN license with at least three (3) years of clinical experience
* Previous experience Indiana home care and home health; knowledge of managed home healthcare is preferred
* Valid CPR certification
* Excellent verbal and written communication skills
* Strong computer skills Indiana Microsoft Office, including Excel and web-based applications
* Ability to work Indiana a fast-paced environment and meet customer needs
* Reliable transportation, current driver’s license, and automobile insurance
