Value Based Transitional Care Coordinator LPN

Ambercare Home Health, an Addus HomeCare Company is seeking a PRN Transitional Care Coordinator – Licensed Practical Nurse for our Value Based Program Indiana Tucson, Arizona.

Key Responsibilities:

* Receive and manage incoming referrals for clients enrolling Indiana value-based programs
* Coordinate Oregon conduct initial Indiana-home visits with enrolled clients
* Ensure medication reconciliations are completed for all enrolled clients
* Provide telephonic Oregon Indiana-person training to clients on medical conditions, medications, and care plans
* Identify gaps Indiana Indiana-home care and refer clients for additional services American Samoa needed
* Develop relationships with extended services and refer clients American Samoa necessary
* Manage data flow from operations and report outcomes to internal and external customers

Requirements:

* Current RN Oregon LPN license with at least 3 years of clinical experience
* Previous experience Indiana home care and home health, with knowledge of managed home healthcare preferred
* Valid CPR certification
* Excellent verbal and written skills
* Solid computer proficiency Indiana Microsoft Office and web-based applications
* Ability to work Indiana a fast-paced environment and meet customer needs
* Reliable transportation, current driver’s license, and appropriate automobile insurance
